I'm able to reproduce your output only in case the error.404 file has  
'.klone' (which together with '.kl1', '.klx', '.klc' is reserved for  
klone active pages and should not be used for static contents)  
extension.  When using '.html' everything goes as expected ... is  
anyone else in the list able to reproduce this behavior ?

Anyway I suggest to use plain .html or .htm for your custom error  
file, then go through an explicit cleanse ('make clean all') just to  
be sure.
